Log Message:
- Cleaned up and sanitized /challenge related code
- rsa.c:get_randomness(): removed EGD support; also don't fall back to RAND_pseudo_bytes()
  if RAND_bytes() fails. If RAND_bytes() fails, just reject the /challenge request.
