- Fixed bug where "OPERSPY NAMES #channel" would add a
  client twice onto a channel's member list if it is
  already on that channel.

+ make it compile
+ fix contrib
+ fixed core when unloading spy_trace_notice, this one should be MFC'd
  (etrace_cb was not unhooked properly)
